The Family Health Nurse Practitioner is responsible for providing medical care for acute and chronic health problems, and annual physical examinations, to patients served by TriValley Primary Care in an office setting. Successful candidate holds a current PA CRNP license in good standing.

Clinical Positions
The Care Manager is a member of a team of professionals dedicated to providing quality healthcare to a patient. The Care Manager is responsible for providing patient outreach and care coordination for patients with moderate to complex medical needs under minimal supervision. Successful candidate holds a current certificate as a Certified Medical Assistant (from the American Association of Medical Assistants), or as a Registered Medical Assistant (from the American Medical Technologists Institute of Education), or holds a current Pennsylvania Nursing License in good standing.

- FT Care Manager Position (LPN/RN) available in our Pennridge Office as well as our Lower Salford Office.
The Office Nurse and Clinical Medical Assistant are members of a team of professionals dedicated to providing quality healthcare to a patient. These clinical team members participate in patient care by rooming patients, collecting vital signs and reconciling medications, assisting the physician, performing various nursing tasks, and acting as a communication link between the patient and the physician and other healthcare providers in the community. Our clinical team members serve as an integral part of our success in value based care by ensuring patient care gaps are addressed during the patient’s visit. Successful candidate holds a current PA nursing license (RN or LPN) in good standing, or holds a certificate as a Certified Medical Assistant or Registered Medical Assistant. The Clinical Supervisor is a member of a team of professionals dedicated to providing quality healthcare to a patient. The Clinical Supervisor is responsible for duties such as: maintaining staff work schedules, evaluating personnel, training & orienting new employees, preparing & monitoring compliance with clinical department budget, recommending changes to work flows and processes, communicating effectively with the public, staff and providers. The Clinical Supervisor will also be responsible for reviewing quality metrics and ensuring proper steps are being taken, as well as nursing duties such as: performing EKG’s as needed, vaccines, updating patient charts, rooming patients as needed, etc.
Successful candidate will possess leadership skills and the ability to work well individually and as a team. Candidates should also be a go-getter and a self starter. Applicants must have background clearances. A minimum of 2 years experience in a busy family practice is required.

Administrative Positions
The Office Manager is and administrative team member who oversees both the admin and clinical teams within the office. The position works closely with the physician shareholders to implement, manage, and monitor various initiatives within the office. Responsibilities include (but are not limited to) scheduling team members, approving payroll and PTO, monitoring overtime, training team members, hiring, firing, and maximizing efficiencies within the office. A successful candidate has experience within primary care, has some knowledge of value based primary care, and excels in a fast-paced environment.

The Population Health Specialist works with our clinical and administrative teams to develop, implement and monitor workflows in the office to achieve desired outcomes as identified by the various payor value-based contracts. The population health specialist participates in office meetings, working with providers and team members to improve efficiencies and continue to educate teams in various initiatives.

Administrative Team Members have a variety of roles within our organization. Duties include (but are not limited to) checking patients in upon arrival, answering telephones to schedule appointments or record detailed messages for the clinical teams, checking patients out, managing in-office medical records requests, and always providing the patients with an excellent experience of care. Administrative team members may be tasked with billing or assisting with population health activities within the office. TriValley offers both Full Time and Part Time positions within our Administrative Teams. Positions usually include rotational Saturday morning hours.

CENTRAL BUSINESS OFFICE POSITIONS:
Patient Collections/Accounts Receivable is responsible for collecting outstanding patient balances. Patients will usually receive 3-4 statements before our patient collections specialist initiates contact to collect payment. Collection attempts are performed via the patient portal, telephone, and by mail. This is an in-person, full time position, Monday through Friday.
